This agreement releases TC Baseball Inc. (The Club Baseball, Clubbies Camps, Clubbies Party) from all liability relating to injuries that may occur during activity. I agree to hold TC Baseball Inc. entirely free from any liability, including financial responsibility for injuries occurred. I understand and acknowledge the risks involved in participating in Baseball and other physical activities.
In consideration of being allowed to participate in the event or activity referenced above, I acknowledge, appreciate, and agree that:
1) The risk of injury from the activities involved in the TC Baseball programs are apparent, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2) I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation; and,
3) I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless the Releasees, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("releasees"), with respect to any and all injury, disability, death or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.
5) I hereby grant permission to TC Baseball Inc. to use photographs, video, and/or any other form of content of my child in any publications, news releases, online, and in other communications related to the mission of TC Baseball Inc. henceforth.
I have read this release of liability and assumption of risk agreement, fully understand that I have given up substantial rights by initialing it, and initial it freely and voluntarily without any inducement.

By Entering my name in this box, I agree to the terms of this waiver.
I further acknowledge that TC Baseball has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that TC Baseball can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other members and their families.
I voluntarily seek services provided by TC Baseball and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending.
I attest that:
* I nor my family are not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I nor my family have not traveled internationally within the last 14 days.
* I nor my family have not traveled to a highly impacted area within the United States of America in the last 14 days.
* I nor my family do not believe we have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I nor my family have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I and my family are following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold TC Baseball and field/facility providers harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of TC Baseball, or that may otherwise arise in any way in connection with any services received from TC Baseball. I understand that this release discharges TC Baseball from any liability or claim that I, my heirs, or any personal representatives may have against TC Baseball with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from TC Baseball. This liability waiver and release extends to TC Baseball together with all owners, partners, employees and volunteers.
